Are you worried about your health and well-being? Caring for a loved one from a distance, or your family lives out of town? Do you or your loved one need just a bit of oversight to help manage health?  You don't have to do this alone. Our Senior Care Wellness program is specifically tailored for seniors, age 55 and older; for those who wish to continue living as independent as possible, for as long as possible, in their home or community setting.   

Our Senior Care Wellness package is developed based on a client's medical and non-medical individualized needs to promote and maintain independence and well-being. 

You can find peace of mind by enrolling yourself or a loved one in this program that provides wellness checks, by a licensed nurse, on a regularly scheduled basis, or as needed. 

Services include:

Head-to-toe physical nursing assessment

Home safety assessment and recommendations

Administration and management of prescription medications as ordered by a licensed provider

Vital Signs monitoring

Skilled nursing services as ordered by a licensed provider 

Nurse companion for doctor’s appointments

Communication with your caregiver, family, and licensed medical provider

Coordination of clinical supplies, such as durable medical equipment 

Coordination & collaboration of community resources, such as home health, hospice

Assistance with activities of daily living, meal prep, errands, and more. 

* Transportation to medical appointments (clients must be able to ride in a standard passenger vehicle and transfer with assist of one)
