Our Senior Care Wellness program is designed specifically for individuals aged 55 and above who aspire to maintain as much independence as possible, whether in their own home or within their community.
Tailored to meet both medical and non-medical needs, our Senior Care Wellness package is customized to promote and sustain independence and overall well-being. Enrolling in this program offers you or your loved one the assurance of regular wellness checks conducted by a licensed nurse, scheduled consistently or as required, providing a sense of security
SERVICES INCLUDE

Head-to-toe physical nursing assessment

Home safety assessment and recommendations

Administration and management of prescription medications as ordered by a licensed provider

Vital Signs monitoring

Skilled nursing services as ordered by a licensed provider

Nurse companion for doctor’s appointments

Coordination of hospital stays and discharges

Communication with your caregiver, family, and licensed medical provider

Coordination of clinical supplies, such as durable medical equipment

Coordination & collaboration of community resources, such as home health, hospice

Assistance with activities of daily living, meal prep, errands, and more.

* Transportation to medical appointments (clients must be able to ride in a standard passenger vehicle and transfer with assist of one)
